The procedure for inspection is straightforward — remove the oil control valve and test it for operation outside the engine manually by applying 12 volts to the terminals of the oil control valve.
A visual inspection of the valve is also important. The valve from this Camry functioned when manually tested the concern can be intermittentbut a visual inspection reveal debris stuck in the bore of the oil control valve proper.
Debris always requires further investigation. The guest allowed the technician to some additional time to inspect the oil feed to the oil control valve, hoping to find the cause of the debris. The oil feed to the oil control valve and VVT system flows through the valve cover, which had no debris:. The cam cap was also removed to inspect the oil feed from the cylinder head, for which there is an oil screen prior to the VVT system, under the circled cap in the below photo when the cap is removed:.
No debris was found anywhere else but in the oil control valve. The timing of the engine was also checked with the valve cover off and was OK and a coincidental damaged tab on a chain guide was also found and repaired.
Per the only inspected related failure being in the oil control valve, it was recommended that the oil control valve be replaced as the first repair attempt. The rattle noise did not change.
When recommending a repair on a complex system, it is always important to discuss with the guest the logic behind the recommended repair and any future repairs or additional diagnostics that may be required. In this particular case, the guest was made aware of the debris in the oil control valve, the unknown source of the debris, and the potential for other VVT issues related to other components particular to the VVT camshaft gear.
With the code persisting, the next step was to address the workhorse of the system as the potential cause of the failure — the VVT camshaft gear. The oil control valve allows oil into and out of the VVT camshaft gear to advance or retard the intake camshaft timing. An internal failure was the next likeliest malfunction, doubly so due to the cold start rattle that often accompanies VVT camshaft gear issues.
Unfortunately, the VVT camshaft gear is not a serviceable part — it cannot be disassembled and reassembled reliably nor inspected. With this being the case, the guest and technician agreed to remove and replace the VVT camshaft gear as the next logical repair attempt. The failure was obvious when the part was disassembled, with one entire lobe of the internal camshaft phaser was broken off and wedged into the gear:.

It's less than a minute IIRC. If you don't do this you may leave excess fuel in the manifold which could cause the knocking when you start up next time as it's overfuelling. The new manifold sorted mine. I experienced the knock a few times, then it was upgraded and never happened since. I do believe there's a software patch to stop it too. I trust you have a full Toyota service history where they can apply these patches? If you don't do this you may leave excess fuel in the manifold.
Without going into the technicalities it is due to the car being shut down during the first couple stages of the warm up cycle in cold damp weather, often when it has been sat for a few days. As a cheap 'fix' it is easier to use EV if you're just going to move the car 20 feet off your drive etc.

Glad it's all sorted. It Seems if you thought it was fixed earlier it was not ,you would have thought Toyota would have contacted owners of the Prius cars that had the 1st modified manifold fitted ,to advise them that a later version was available.
Ok if you let it warm up before you switch it off after a cold start it may not happen ,Prius was engineered for economy ,not to waste fuel warming up a cold engine. Only thing bothers me is the damage explosions do in the exhaust ,a Catalytic converter may be damaged and looking at it, it does not look cheap!
Isn't it also partly software? I believe there are a number of different versions out there and varieties thereof. This became apparent on the US Priuschat forum when the inverter recalls were in full swing.
Some owners experienced severe retardation of acceleration following the recall and others didn't. Upon investigation it turns out there were different software versions in use, some allowing harsher use of the inverter. Once updated to the latest versions throughout, owners have had happy cars that start fine on cold damp mornings. Well, so long as they got the updated manifold aswell.

The odd thing is that I can disconnect the spark plug wire on any cylinder and it will run fairly smooth.

It can take ten minutes or more to get the car started. This happens when the car is cold, or has been sitting for a while, or randomly at other times. The Toyota service department will only address the problem if they can replicate it. They reset a computer component which helped for three months.
Any solutions that you know of? You might post this on a Prius forum, like Priuschat. There will be a large number of Prius owners that can help, and maybe this question has already been asked. Have they checked your 12V battery? Observe the start button as you press the brake when starting. The light should change from orange to green. If the light in the start switch is flashing this indicates there is too much tension on the steering system to release the steering lock.
This can occur when one front tire pressed against a curb or parking stop. If this is the case turn the steering wheel side to side to remove tension from the steering lock. From my experience with batteries, I start looking for battery trouble at the four year mark.
So, rather than spending a lot of time trying to diagnose the problem, I replaced the battery and then waited to see if the lack of the READY light problem continued. It did not. My second comment relates to the prices of the special type of battery needed for the Prius.
